Medical Records Request

If you need to request your medical records, please call 828.315.3115, select Option 1, and you will speak to the Release of Information Department.  They will be able to provide you with instructions on how to obtain an “Authorization for Release of Information” form or you can download and print it below. 

If you would like to request your medical records in person, Hours of Operation are: Monday – Friday, 8:00 am – 4:30 pm. Closed on weekends and Holidays.
